That translates into his showing pseudo-"interest" in a guy (and traumatizing the guy), and giving a character he knows is crossdressing some awkward grief before the character caves and shows he's a guy. There is also another section where Misaki is attacked by two perverts and tied up. They are thwarted before anything happens though.
That's all the innuendo I remember, there could be more. I personally found Maid-Sama! to be hysterical, and Usui is really sweet a lot of times (he's kind of an enigma). But there is a slightly higher notch than Ouran in the innuendo category, if that's a useful comparison at all.
